ABSTRACT

Acupoint sticking therapy is a comprehensive Traditional Chinese Medicine treatment method combining meridian, acupoint and Chinese herbal medicine. It is widely used in the treatment of chemotherapy-related gastrointestinal reaction, cancer pain, tumor related intestinal obstruction, cancer-related fatigue, etc. It is oftenuse in conventional treatment and nursingprocess, or usedalone, or combined with oraladministration of Chinese and Western Medicine and acupoint massage. This therapy could avoid the gastrointestinal irritation caused by oral administration. The advantages are as follow: the operationis simple with obvious effect, and low price andtheacceptance is high.

ABSTRACT

Objective:To investigate the effects of external application of modified Xiangsha Liujunzi Tang in improving gastrointestinal reaction, immune function, and quality of life in patients with gastrointestinal tumors after moderately/highly emetogenic chemotherapy. Method:A total of 140 inpatients (from January 2018 to May 2021) with gastrointestinal malignancies diagnosed by the Oncology Department of Integrated Traditional Chinese and Western Medicine of China-Japan Friendship Hospital and treated with moderately/highly emetogenic chemotherapy were randomly divided into an experimental group (n=70) and a control group (n=70) according to the Good Clinical Practice (GCP). Participants were given routine antiemetic treatment once 20 minutes before chemotherapy (tropisetron 5 mg + dexamethasone 5 mg or methylprednisolone 40 mg). On this basis, the patients in the experimental group were treated with external application of modified Xiangsha Liujunzi Tang for 1 week on the second day after chemotherapy (the selected points were Zhongwan, Neiguan and Zusanli, 6 hours a day, and the application was changed every day), and the patients in the control group were applied with comfort patch at the same time. The gastrointestinal reaction grade, subset concentration of lymphocytes, and Karnofsky score in two groups before and one week after chemotherapy were recorded, and all data were analyzed by SPSS 19.0 statistical software. Result:The grades of chemotherapy-related gastrointestinal reaction in two groups after treatment decreased as compared with that before treatment (P<0.05). The experimental group was superior to the control group after treatment in terms of grade decrease (P<0.05). The total response rate of the experimental group was 81.43% (57/70), higher than 62.86% (44/70) in the control group (χ2=9.73, P<0.05). Stratified analysis was performed on the experimental group. Compared with the conditions before treatment, the grade of gastrointestinal reaction in patients with esophageal cancer and colorectal cancer of the experimental group decreased after treatment (P<0.05), and that in patients of different genders decreased after treatment (P<0.05). The results of immune function showed that there was no significant difference in the concentrations of CD3+, CD4+, CD8+, and NK cell subsets of the control group before and after treatment, while the concentrations of CD3+, CD4+, and NK cell subsets of the experimental group were higher than those before treatment and even superior to those in the control group (P<0.05). The Karnofsky score of quality of life in the experimental group was higher than that in the control group, with quality of life improved (P<0.05). Conclusion:External application of modified Xiangsha Liujunzi can improve the gastrointestinal reaction, immune function, and quality of life of patients with gastrointestinal tumors after moderately/highly emetogenic chemotherapy.

ABSTRACT

OBJECTIVE: To observe the effect of warming needling therapy on gastrointestinal reaction after hyperthermic intraperitoneal chemotherapy (HIPEC) in patients of spleen and stomach deficiency syndrome after colon cancer surgery. METHODS: A total of 120 cases of HIPEC were randomized into observation group and control group, 60 cases in each. The patients of the two groups all received HIPEC. In the observation group, 1 h before HIPEC, warming needling technique was applied to Zusanli (ST36), Sanyinjiao (SP6) and Yinlingquan (SP9) and the even-needing technique of acupuncture was applied to Neiguan (PC6) for 30 min, and then the intravenous injection with Ondansetron was given 30 min before HIPEC. In the control group, The intravenous injection with Ondansetron was given 30 min before HIPEC. In the two groups, the changes in nausea, vomiting, abdominal distention, diarrhea, total bilirubin (TB), alanine aminotransferase (ALT), alkaline phosphatase (ALP) and KPS score, as well as the average hospitalization length of stay were observed. RESULTS: The total effective rates in the treatment of nausea, vomiting, abdominal distention and diarrhea were 91.67%(55/60), 93.33%(56/60), 80.00%(48/60) and 88.33%(53/60) in the observation group and were 78.33%(47/60), 78.33%(47/60), 63.33%(38/60) and 70.00%(42/60) in the control group respectively, and the total effective rate in the treatment of gastrointestinal reaction of HIPEC in the observation group was obviously higher than that in the control group(P0.05). CONCLUSION: The warming needling technique of acupuncture and moxibustion alleviates the gastrointestinal reaction, improves KPS score and reduces the hospitalization length of stay in HIPEC patients after the surgery of colon cancer and differentiated as the spleen and stomach deficiency in traditional Chinese medicine.

ABSTRACT

OBJECTIVE:To evaluate the safety of Tripterysium glycosides tablet systematically, and to provide evidence-based reference.METHODS:Retrieved from Chinese Journal Full-text Database,China Science and Technology Journal Database,Wanfang database,PubMed,Embase,Medline (Ovid) and The Cochrane Library, randomized controlled trials (RCTs) about added or combined with Tripterysium glycosides tablet (trial group) versus basic treatment or combined with other drug (control group) in the treatment of various disease-induced ADR (the incidence of leukopenia, gastrointestinal reaction, irregular menstruation, lover dysfunction) were collected. After data extraction and quality evaluation with Cochrane 5.1.0 quality evaluation tool,RevMan 5.0 statistical software was used to conduct Meta-analysis.RESULTS:A total of 36 RCTs were included, involving 2 814 patients. Results of Meta-analysis showed that the incidence of leucopenia [OR=3.42,95 % CI (1.88,6.22),P< 0.001],irregular menstruation [OR=8.42,95%CI(4.93,14.38),P<0.001] and abnormal liver function [OR=2.99,95%CI(2.01,4.46),P<0.001] in trial group were significantly higher than control group, with statistical significance. Results of subgroup analysis showed that the incidence of irregular menstruation in trial group was significantly higher than control group, with statistical significance [OR=5.39,95% CI (2.50,11.59),P<0.001].When combining with glucocorticoids, the incidence of gastrointestinal adverse reactions in trial group was significantly lower than control group, with statistical significance [OR=0.40,95%CI(0.19,0.84),P=0.02].When the indications were the diseases of immune system, the incidence of leucopenia [OR=3.42,95% CI (1.49,7.82),P=0.004] and abnormal liver function [OR=2.27,95% CI (1.08,4.76),P=0.03] in trial group were significantly higher than control group, with statistical significance.CONCLUSIONS:ADR induced by Tripterysium glycosides tablet mainly include leucopenia, gastrointestinal reaction, irregular menstruation and abnormal liver function.More attentions should be paid on irregular menstruation when combining with other drugs.More attentions should be paid on leucopenia and abnormal liver function when treating the diseases of immune system.
